Registration Policies:
  • In order to reduce our environmental footprint, printed copies of the syllabus will not be made available for each participant this year. Instead, a single copy will be placed on each table in the ballroom and posted on PowerPoint slides. The program at a glance will also be included in each delegate bag. If you would like your own personal copy of the syllabus please print and bring with you from the Palliative Institute website.
  • A $40 administration fee will apply per transaction to delegates with substitutions & cancellations. If you wish to cancel please contact our office
  • The registration fee is non-refundable after September 30, 2016.
  • Registration fees must be paid in full before you can be admitted to the conference. 
  • Payment can be made by Visa, Mastercard or AMEX.
  • You will receive a confirmation/receipt email after your registration has been processed.
  • Registration will be closed at midnight on October 21, 2016.

One Day $225 Early Bird Rate / $250 Regular Rate after September 16, 2016
Two Day $450 Early Bird Rate / $500 Regular Rate after September 16, 2016
